If I make a decision to talk to you on the phone, I will ask you on the phone to give me a definite yes or no decision while on the phone together. I only work with a small percentage of people that fill out this questionnaire either because they are not in the income class that this will benefit, or something is said in this questionnaire form that I feel it would be a waste of time even talking about the matter.
